Let Advocates Connect give you and your loved on the peace of mind that comes from knowing someone is in regular contact and reporting any care concerns.
What we offer
Check in calls: Three times per week, a member of our care team will call your loved one to make sure all is well and to see if there are any concerns that need to be addressed. We will follow up on any concerns by either contacting a designated family member/friend or taking care of the issue if it is in the scope of services we can provide (e.g. picking up a prescription, helping to make an appointment or arranging for grocery delivery).
In person visits: Every other week, a member of our care team will stop in for a chat and do a visual assessment of your loved one’s health and environment. This will be an opportunity to review any safety concerns such as checking the fridge for outdated food items, looking for fall risks and making sure personal hygiene and nutritional needs are being met.
Contact summaries: After each contact, call or visit notes will be available in an on-line portal with access for those who have been given permission.
Peace of Mind – We are watching out for the well-being of your loved one; providing companionship and conversation to combat loneliness and keeping eyes on the situation to monitor health and safety.
Information – Online access to summaries of visits and phone calls, along with resources throughout the local community.
Maintaining Independence – Your loved one will be able to stay in his/her own home with minimal intrusion, a local presence for on-going support, and assistance when needed.
